How to differentiate it from sperm agglutination syndrome
Sperm agglutination is caused by the agglutination reaction between sperm antigens and sperm antibodies, resulting in sperm clumping together head to head, tail to tail, or head to tail. Sperm malformation is characterized by abnormal individual sperm morphology, such as large heads, small tails, double heads, double tails, etc., with a malformation rate exceeding 30%.
How does Western medicine treat this?
1. Anti-inflammatory treatment: Cephalosporins, erythromycin, norfloxacin, fluoxetine, carbenicillin, etc. can be used. Rifampin and trimethoprim can also be used in combination. Rifampin 300 mg and trimethoprim 80 mg, one tablet in the morning and two tablets at night, for 15 days; then two tablets each night for 15 days; then one tablet each night for 90 days.
2. Zinc therapy: 100-200 mg of zinc sulfate each time, 3 times a day, for 3-5 consecutive days.
3. Vitamin therapy: Take one vitamin E capsule once daily orally.

How does Traditional Chinese Medicine differentiate syndromes and treat diseases?
1. Kidney essence deficiency type: Symptoms include infertility after marriage, sperm abnormalities, soreness and weakness of the lower back and knees, dizziness and tinnitus, forgetfulness and insomnia, decreased libido, pale tongue, thin white coating, and deep and thready pulse.
Treatment principle: Tonify the kidneys and replenish essence.
Prescription: 20g Rehmannia glutinosa, 12g Cornus officinalis, 15g Dioscorea opposita, 12g Lycium barbarum, 15g Cuscuta chinensis, 10g deer antler glue (melted), 10g tortoise shell glue (melted), 12g Angelica sinensis, 6g Schisandra chinensis.
2. Qi and Blood Deficiency Type: Symptoms include infertility after marriage, abnormal sperm, sallow complexion, fatigue, palpitations, shortness of breath, dizziness, poor appetite, loose stools, pale tongue with white coating, and weak pulse.
Treatment principle: Tonify Qi and nourish blood.
Prescription: Astragalus membranaceus 20g, Codonopsis pilosula 15g, Angelica sinensis 15g, Rehmannia glutinosa 15g, Paeonia lactiflora 12g, Ligusticum chuanxiong 6g, Atractylodes macrocephala 10g, Poria cocos 10g, Lycium barbarum 12g, Cuscuta chinensis 15g, Glycyrrhiza uralensis 6g.
3. Damp-heat accumulation type: Symptoms include infertility after marriage, thick or non-liquefied semen, increased abnormal sperm, bitter and sticky taste in the mouth, damp scrotum, incomplete bowel movements, red tongue with yellow and greasy coating, and slippery and rapid pulse.
Treatment principle: Clear heat and promote diuresis.
Prescription: 20g Dioscorea hypoglauca, 30g Coix lacryma-jobi, 20g Poria cocos, 15g Plantago asiatica (wrapped), 15g Dioscorea opposita, 10g Atractylodes macrocephala, 10g Phellodendron chinense, 20g Cistanche deserticola, 15g Achyranthes bidentata, 15g Cuscuta chinensis.
Traditional Chinese medicine prescriptions for treating sperm abnormalities
1. Equal parts of fish maw glue, Astragalus complanatus seed, Cuscuta chinensis seed, Lycium barbarum fruit, Epimedium brevicornu, Impatiens balsamina seed, and Eucommia ulmoides bark. Grind all the above herbs into a fine powder, mix with honey to form pills, each weighing 9 grams. Take one pill twice a day, morning and evening. Drink 200 ml of lightly salted water in the morning and 50 ml of rice wine in the evening. One month constitutes one course of treatment.
2. Take 10 grams of Schisandra chinensis, 15 grams of Lycium barbarum, 15 grams of Cuscuta chinensis, 20 grams of Rubus idaeus, and 15 grams of Plantago asiatica (wrapped). Decocted in water and taken once a day.
3. Take one fresh human placenta, 100 grams of wolfberries, and 100 grams of leek seeds. Dry them in a drying oven, grind them into a fine powder, and take 10 grams each time, twice a day.
Medicinal diet for treating sperm abnormalities
1. 5-10 sparrows, 100g of japonica rice. Remove the feathers and internal organs of the sparrows, wash them and stir-fry them until cooked. Then add a small amount of white wine, simmer briefly, add water, add the japonica rice and cook into porridge. When the porridge is cooked, add 3g of scallion whites and cook for a few more minutes. It can be served as breakfast, dinner or snack in winter.
2. 6 grams of Cordyceps sinensis, 12 sparrows, 2 grams of ginger, 2 grams of scallions, and salt to taste. Remove the feathers and internal organs of the sparrows, wash them, and cut them into pieces. Place the Cordyceps sinensis, sparrow pieces, scallions, ginger, and salt into a clay pot, add enough water, bring to a boil over high heat, then simmer over low heat until the sparrow meat is tender. Eat the meat and drink the soup, including the Cordyceps sinensis, in several servings.
3. Stew 250 grams of deer antlers, appropriate amounts of Chinese cabbage and vermicelli together until cooked and serve.
4. Boil 15 grams of earthworm and 2 eggs together for 15 minutes, then crack the eggshells and boil for another 10 minutes. Eat once every 2 days.
5. 50 grams of walnut kernels and an appropriate amount of chives. First, fry the walnut kernels in sesame oil until golden brown, then add the washed and chopped chives, stir-fry, season with salt, and eat as desired.
6. One male duck and 5-10 pieces of cordyceps. Remove the feathers and internal organs from the male duck, wash it clean, put it in a casserole, add cordyceps, salt, ginger, scallions and other seasonings and an appropriate amount of water, simmer over low heat, eat the meat and drink the soup.
What should we pay attention to in daily life?
1. Avoid spicy foods, and abstain from smoking and alcohol. Pay attention to personal hygiene, especially the hygiene of the external genitalia.
2. Treat epididymitis, orchitis, seminal vesiculitis, varicocele and other diseases in a timely manner.
3. Avoid wearing tight pants, stay away from high-temperature environments, and avoid strong light and radiation stimulation.
pyospermia
Pyospermia refers to the presence of pus cells in semen or a white blood cell count of more than 5 under high power, or the ejaculation of purulent semen. It is a common condition of male infertility.
Causes of disease
When the reproductive organs are infected with bacteria, such as in cases of suppurative inflammation like seminal vesiculitis, epididymitis, or prostatitis, the secreted semen contains a large number of pus cells, which are expelled with the semen, leading to pyospermia or excessive white blood cells. Simultaneously, inflammation of the reproductive system can affect sperm production, resulting in oligospermia, necrospermia, or teratospermia. Furthermore, a large number of pus cells can cause abnormal seminal plasma composition, leading to poor liquefaction and increased viscosity, resulting in low sperm motility and survival rate, ultimately causing infertility.
What are the symptoms?
In patients with pyospermia, the semen not only contains clumps of pus cells but also has a prolonged liquefaction time. Patients experience painful urination, yellow urine, mild ureteral pain during ejaculation, or lower abdominal discomfort. The semen is slightly yellow and purulent to the naked eye. Stools are often dry and hard. There is tenderness upon palpation of the perineum, or pain during digital rectal examination and prostate massage.
After receiving the patient's semen, most women experience abdominal pain, which usually subsides after 2-3 hours.
This disease often coexists with seminal vesiculitis, prostatitis, or epididymitis.
What tests should be done?
1. Routine semen analysis: The semen appears yellow or purulent, with an elevated pH value, prolonged liquefaction time or no liquefaction at all, and a large number of white blood cells or pus cells can be seen under a microscope, more than 5 per high-power field.
2. Examination of prostatic fluid: Increased white blood cells and decreased lecithin bodies were observed.
3. Semen bacterial culture: Pathogenic bacteria can be cultured.
